One per team must be submitted before the first games. Used to ensure that all parents understand what is expected during the season.

Suggested for each player on U10 and older teams. Complete this sheet at the beginning of each season and keep on hand at games.

For players living in Waxahachie who wish to play for a different association.

For players wishing to play on an academy team. Submit form and birth certificate to: registrar@waxahachiesoccer.org and allow two weeks turn-around.

For coaches/players to verify that a required sit-out occured.

For complaints and game protests. Instructions included with form.

For requesting a refund of player registration fees.
